Numerous review articles and clinical practice guidelines currently advocate low dose anticoagulation with warfarin for most patients with chronic non-valvular atrial fibrillation (CNVAF). This recommendation is based on five randomized controlled trials (RCTs), the results of which are generally interpreted as showing a clear reduction in the risk of stroke, with an acceptable profile of side effects from treatment. Based on our review of the evidence, including a critical appraisal and meta-analysis, we conclude that it is not. On balance, the margin between expected benefit and harm for warfarin prophylaxis in patients with CNVAF is unfortably thin. The low absolute risk reductions observed in RCT's would likely be overwhelmed in less controlled settings by problems associated with the use of warfarin. Furthermore, the methodological limitations of the original five study designs and the analyses used are likely to have produced inflated estimates of the true risk profile. The results of the RCTs have been presented in misleading ways, in particular 1) relative risk reductions have been widely highlighted, despite low event rates and absolute effect sizes; 2) limits on external validity have not been acknowledged, although only 6-8% of patients with atrial fibrillation were actually entered into the studies, and 3) that the efficacy of warfarin is likely to be substantially reduced outside ideal study settings.
