An important part of the work has also been to review the service recommendations and training guidelines produced by the Royal Colleges and professional associations and to assess their implications for concentration. The results of this work suggest that there is no compelling reason to believe that further concentration of hospital services will result in improved efficiency (through exploiting economies of scale) or to automatic improvements in the quality of clinical outcomes. In assessing the potential negative effects of increased concentration on access and utilisation, the implications for disadvantaged groups in particular should not be overlooked. Even where specific effects on costs, outcomes or utilisation can be demonstrated in the literature, the process by which such effects are generated is poorly understood.
