This study investigated the extent of heterotypic continuity of psychiatric disorder (i.e., one class of disorder predicting another class of disorder) during the transition from adolescence to adulthood, taking into account homotypic continuity (i.e., continuity in the same class of disorder) and concurrent comorbidity. Lifetime prevalence of DSM-III-R anxiety, depressive, behavioral, and substance use disorders by the participants' 19th birthday was used to predict either the recurrence or new onsets of these same classes of disorder during the next 4 years. Significant levels of heterotypic continuity were found for 75% of the possible pairs of disorder when homotypic continuity and concurrent comorbidity were not controlled in the analyses. However, with these controls in the prediction equations, only 50% of the possible pairs demonstrated significant heterotypic continuity. With controls in place, anxiety predicted later depression, and vice versa. Substance use disorder predicted later behavioral disorder, and vice versa. Earlier substance disorder predicted later depression, and earlier depression predicted later behavioral disorder. All four classes of disorder demonstrated clear and strong homotypic continuity regardless of the controls implemented. The results of this study suggest that there is a good deal of heterotypic continuity, but that its prevalence is inflated when homotypic comorbidity and concurrent comorbidity are not taken into account.
