In September 1980 the Merchant Vessel Derbyshire disappeared without trace during a violent storm in the Pacific about 350 miles south east of Japan. She was an oil/bulk/ore carrier, and the largest British ship ever lost at sea. All those on board perished including 42 members of crew and two of their wives. On paper the Derbyshire was a young ship with a good safety inspection record and an experienced crew. A formal investigation between 1987-88 produced inconclusive findings partly because the exact location of the wreck was still unknown. In 1994 an independently financed search located the Derbyshire, and an official survey was ordered. The subsequent report from the appointed assessors imputed negligence on behalf of the crew but also acquitted the design and construction of the vessel. In 1998 the Government announced that the formal investigation would be reopened and held in the High Court. This report presents the findings of the investigation conducted between April and July 2000. The most far-reaching recommendations included in this report concern the design and the international specification regulations of the compartment hatches on bulk carriers. The report concludes that the failure of such a hatch was the most likely cause for the sinking of the Derbyshire. The findings in this case reverse those arrived at in 1994, and exonerates the Derbyshire's crew of poor seamanship. This report includes sections covering all aspects of the evidence and research covered in this case. An accompanying CD-ROM gives access to full transcripts of the case hearings as well as to animations of simulation tests carried out on models of the ship.
