From an early age Jacob's life had an air of inevitability about it, following the mysterious disappearance of his parents, and he begins to spiral out of control. After another life-changing setback leads to a deterioration in his already fragile mental state, Jacob finds himself placed into the care of the Winebeck Hospital for the Insane. During his time there, he endures years of physical and mental abuse, at the hands of a group of hospital guards, psychiatric nurses and fellow patients. He is also witness to an appalling catalogue of gross injustices, against other patients, by some of the very same individuals who made his life such a misery. Finally, at the age of 27, Jacob is discharged, and free to return to his hometown of Pentonville, in the southern state of Mississippi. The decision to release him, on compassionate grounds, comes about after Jacob is diagnosed with a terminal illness. After reviewing his case, Dr Halpin, one of very few staff members with any form of empathy, persuades the board of directors to allow Jacob to finish the remainder of his life outside the bleak, unforgiving walls of the hospital. But Jacob is haunted by the pure evil he has had to contend with over his years of enforced incarceration. And the nightmares and dark memories still burn deep within his soul. A short time after his release, a chance sighting of some of the hospital guards who had treated him so badly, culminates in Jacob setting out on a vengeful and murderous course of action, on those who had brutally invaded his world.
