Sacral nerve stimulation for the treatment of refractory urinary urge incontinence or urinary retention involves the application of electrical stimulation to the sacral nerve via a totally implantable system which consists of an electrode placed extradurally close to the third sacral nerve, an implantable pulse generator and an extension which connects the lead to the pulse generator. Up to 200 Australian patients per year are expected to be candidates for a sacral nerve stimulator implant. There is a relatively high incidence of adverse events associated with sacral nerve stimulator implants. The best available evidence is provided by two RCTs, both of which have methodological deficiencies which limit the strength of their results. Sacral nerve stimulation appears to have treatment benefits for female patients with urge incontinence in terms of reducing the incidence of urinary incontinence, reducing pad consumption, and regaining of bladder control. The impact on the quality of life of patients is uncertain. The indicative cost-effectiveness ratio for sacral nerve stimulation treatment for urge incontinence is estimated to be approximately $35,000 per additional patient free of continence at six months follow-up. MSAC found that on the strength of the evidence pertaining to sacral nerve stimulation: the intervention is associated with a relativelv high rate of adverse events; the long-term effectiveness is uncertain; and the cost-effectiveness ratios associated with the intervention are unfavourable. MSAC therefore recommended that public funding for sacral nerve stimulator implantation should not be supported at this time.
